Remember your membership card

You will have a membership card sent to you with all the contact details you will need, please keep this with you, if you require medical treatment you will need to show your card to your healthcare provider and they will then liaise with Olympus directly to organize payment. When you call Olympus all you will need is your policy number to hand.

Step 3: The Provider Visit

Arrive at least 15 minutes prior to your appointment time. When visiting a doctor for the first time, you will be asked to complete a registration form; standard information is asked in this form: demographics, insurance information, present or past medical conditions including medications you may be taking, emergency contacts and person financially responsible for payment (in the USA, any expense incurred not covered by your insurance company are ultimately the members responsibility). Provider’s offices will also ask you to sign a release of protected health information, this signed release will allow your physician to provide medical information to your insurance for pre-authorization and claim processing purposes.

Should the doctor’s office have any questions regarding your coverage, please ask them to contact the AVIVA dedicated Helpline at Olympus (+1) 800.649.0729 (toll-free from the USA) (noted on the back of your member card) for eligibility and benefit verification.  Alternatively, you can contact us and we will gladly speak with the provider to arrange a “assignment of benefits” (Request for the doctor to send the claim directly to us.)
